Great family run restaurant with traditional Tuscan food.

We visit Montalcino once a year and eat many meals each trip at Porta al Cassero. The menu is small and there's really a small number of appetizers and second courses that we like but we keep going back for the fantastic Pinci al Sugo (as well as other sauces for the Pinci) and the Scottiglia - a remarkably lean stewed wild boar. Their soups are also quite good. It's a classic family run place where the father is behind the bar, daughter waiting tables and grandmother in the kitchen. No English spoken unless someone other than the daughter is waiting tables. They also have a decent selection of local wines at reasonable prices.
